Medberry Primary Care is required by law to protect the privacy of your health information. We are also required to give you this notice about our privacy practices, our legal duties, and your rights. We must follow the terms of the notice that is in effect.
How We May Use and Share Your Health Information
We may use and share your health information for the following reasons.
For Treatment. We use your health information to provide care. For example, providers and staff in our clinic may share your information to treat you, order tests, or refer you to a specialist.
For Payment. We use your health information to bill and collect payment. For example, we may share information with your insurance plan to confirm coverage and get paid for your visit.
For Health Care Operations. We use your health information to run our practice. For example, we may use it to review the quality of care, train staff, or manage daily operations.
Appointment Reminders and Follow-Up. We may contact you to remind you of an appointment or to follow up on your care. This may include a phone call, text message, or email.
Others Involved in Your Care. With your permission, we may share information with a family member or friend involved in your care.

You may cancel your authorization in writing at any time. This will not affect information we already shared while the authorization was active.
Your Rights
You have the following rights regarding your health information.
The right to see and get a copy of your health record. We may charge a reasonable fee for copies.
The right to ask us to correct your health record if you believe it is wrong or incomplete.
The right to ask for limits on how we use or share your information. We will consider your request, though we are not always required to agree.
The right to ask us to contact you in a certain way or at a certain place. For example, you may ask us to call you only at work.
The right to a list of certain times we shared your information.
The right to a paper copy of this notice, even if you agreed to receive it electronically.
The right to be told if a breach affects the privacy of your health information.

We are required by law to:
- Keep your health information private

- Follow the terms of the notice currently in effect
- Tell you if a breach affects your unsecured health information
Changes to This Notice
We may change this notice at any time. A new notice will apply to all health information we keep. We will post the current notice in our clinic and on our website.
How to File a Complaint
If you believe your privacy rights were violated, you may file a complaint with us. Contact our Privacy Officer below.
You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
