HRT and Hormone Therapy
in San Antonio, TX
HRT in San Antonio starts with knowing what your hormone levels actually are. Medberry Primary Care provides hormone pellet therapy for men and women over 40, at our clinic on Fredericksburg Road near the Medical Center.
We are a primary care practice, not a med spa. That means your hormones get treated as part of your whole health picture, by the same team that handles your labs, your medications, and your annual physical.
Same-day and next-day appointments. No referral needed. Most major insurance accepted.
Who We Treat With
Hormone Pellets in San Antonio
Pellets are small implants placed under the skin that release hormones slowly over several months. We use EvexiPEL pellets, placed by Adam Casillas, PA-C, at our San Antonio clinic.
Patients usually come to us with some version of the same story. Tired no matter how much they sleep. Short-tempered for no reason. Workouts that used to feel routine now feel like too much. Weight settling around the middle and not moving.
The symptom patterns we see most often:
- Fatigue that does not improve with rest
- Night sweats or hot flashes
- Low sex drive or changes in sexual function
- Irritability or low mood without an obvious cause
- Brain fog and trouble holding a thought
- Weight gain through the midsection that does not respond to diet and exercise
- Loss of muscle or strength despite staying active
If that list sounds familiar, the next step is lab work.

Why Hormone Replacement Therapy Works Better at a Primary Care Practice
Every symptom on that list can also be caused by something other than hormones. An underactive thyroid. Low iron. Sleep apnea. Depression. A new blood pressure medication.
A clinic that only does hormones can only find hormone problems. We can find the others, and we can treat them.
At your first visit we run hormones, thyroid, a complete blood count, and a metabolic panel together. One blood draw. Then we sit down with the results and tell you what is actually going on.
Sometimes the labs point to hormone therapy. Sometimes they point to something faster and simpler to fix. Either way you leave with real answers instead of a guess, and if pellets are right for you, we place them.
That is the difference between a hormone clinic and a full primary care practice. You are not buying a treatment. You are getting a diagnosis first.
Hormone Pellet Therapy Pricing
in San Antonio
Your exact price depends on your labs, your dose, and your coverage. The best way to get a real number for your situation is a quick consultation with a Medberry provider. We walk you through the full breakdown before you commit to anything.
$350 per insertion, typically lasting about 4 months.
$799 per insertion, typically lasting about 6 months.
What Happens During a Hormone Pellet Insertion
The procedure is smaller than most people expect. You are in and out in under thirty minutes, awake and talking the whole time. There is no hospital stay and no sedation.
Here is the sequence:
- Your provider reviews your lab results and symptoms with you.
- You lie on your side. The skin over your upper hip is cleaned and numbed with a local anesthetic.
- A small incision, a few millimeters, is made.
- The pellets are placed in the fat layer under the skin. You should not feel this part.
- The site is closed with adhesive strips or a small bandage. Stitches are usually not needed.
- You leave with written aftercare instructions and a follow-up plan.
The part patients ask about most is the numbing shot. It stings, and that is the only part that does. It passes quickly.
After you go home
Keep the site clean and dry for a few days. No swimming, no baths, no hot yoga. Walking is fine. Hold off on heavy lifting and hard workouts for about a week, or as your provider directs.
Some bruising and tenderness at the site is normal and fades on its own. Call us if you notice spreading redness, increasing swelling, warmth, or drainage.
Same-Day HRT Appointments in San Antonio
Hormone care is not one appointment. It is labs, a visit to discuss the labs, a decision, the insertion, then labs again a few weeks later.
If each of those takes three weeks to schedule, you are two months out before anyone decides anything. That wait is the most common complaint we hear from patients coming from other clinics.
We fixed that first. Call in the morning and in most cases we can get your bloodwork done before you head home. We keep same-day and next-day appointments open daily, we take walk-ins, we are always accepting new patients, and you do not need a referral or an existing relationship with the practice.
Our Fredericksburg Road location near the Medical Center makes repeat lab visits easy. Patients from Balcones Heights, Shavano Park, and around Loop 410 come in on a lunch break.
Who Should Not Start Hormone Pellet Therapy Yet
Pellets are not right for everyone, and they are not right for everyone immediately.
Once pellets are placed they stay under the skin for months and cannot be removed quickly. We would rather have an honest conversation than put something irreversible in someone who is not ready.
We will not place pellets before your labs are done. Fatigue, brain fog, low libido, and low mood have too many possible causes to treat blind.
Tell your provider before your visit if any of these apply:
- You are pregnant, nursing, or trying to conceive
- You have been diagnosed with breast, uterine, or prostate cancer
- You have had a blood clot, stroke, or heart attack
- You have liver disease or unexplained vaginal bleeding
- You are currently on another hormone treatment we have not reviewed
None of these is automatically a no. They mean your provider needs to talk with you before moving forward.
A note on sleep, stress, and mood. Low energy is often tied to poor sleep, chronic stress, or untreated depression and anxiety. Our primary care team treats all of it, and for some patients, addressing those first means pellets are never needed.

Common Questions About HRT
in San Antonio
Pellet insertions are performed by Adam Casillas, PA-C, at our San Antonio clinic. Adam has over 13 years of experience in patient care. Your labs and results are reviewed by the same team that handles your primary care. Hormone therapy carries risks and benefits that depend on your individual history, and your provider reviews those with you before any treatment begins.
We are a primary care practice. The providers placing your pellets are the same ones managing your blood pressure, your medications, and your annual physical. A hormone clinic can only look for hormone problems. We run thyroid, iron, metabolic, and blood count panels alongside your hormone labs, so if something else is causing your symptoms, we find it and treat it.
You will talk through your symptoms and history, then have blood drawn the same day. We test hormones, thyroid, CBC, and a metabolic panel together in one draw. At your follow-up we go through the results and tell you what they show. If pellets are a good fit, we schedule the insertion. If the labs point somewhere else, we treat that instead.
The numbing shot stings briefly. That is the only part most patients feel. After the area is numb you will not feel the small incision or the pellet placement. You stay awake and talking throughout, and the whole visit takes under thirty minutes. Most patients tell us afterward that it was smaller than they expected.
You go home with a small bandage. Keep the site clean and dry for a few days, and skip swimming, baths, and hot yoga. Walking is fine. Avoid heavy lifting and hard workouts for about a week. Bruising and soreness are normal and fade. Call us if you see spreading redness, swelling, warmth, or drainage.
The pellets themselves are self-pay. Your office visits and lab work are often billed through insurance, because we are a primary care practice rather than a med spa. We accept Blue Cross Blue Shield, UnitedHealthcare, and Aetna commercial plans, plus Blue Cross Blue Shield Medicare Advantage. Coverage varies by plan, and our front desk will verify your benefits before you schedule.
Pellet insertions are $350 for women and $799 for men, priced per insertion and lasting several months. Your total also depends on your labs, your dose, and your insurance coverage. We give you the full picture in a quick consultation before you schedule anything.
Yes. We will not place pellets before your labs are complete, because too many conditions produce the same symptoms. We also slow down if you are pregnant or trying to conceive, have a history of breast, uterine, or prostate cancer, have had a blood clot, stroke, or heart attack, have liver disease or unexplained vaginal bleeding, or are on another hormone treatment we have not reviewed. None of these is an automatic no. They mean a conversation comes first.
